New Patient Information

At Landmark Dental Group, your first appointment includes a thorough exam and gathering any information that our dentist must be aware of in order to provide care specifically to your oral health. We understand that all patients are different and require individualized care. 




Financial policy

Our practice is committed to providing the best treatment for our patients and we charge what is usual and customary for our area. Your insurance policy is a contract between you, your employer, and the insurance company. We are not a party to that contract and there is nothing we can do regarding the coverage provided. As dental health providers our relationship is with you, not your insurance company. Not all services are a covered benefit in all contracts and some companies arbitrarily select services which they exclude.

If you have dental insurance, we will gladly submit your claim to your dental insurance company for you. At the time of your appointment, you will be billed for your dental care; if you have dental insurance, your copay, the amount insurance tells us they will not pay, is due at the time of service. If you do not have dental insurance, you will be billed the total amount at that time. We accept cash, check, or the following credit/debit cards: Visa, Mastercard, Discover and American Express.

All charges incurred are ultimately the responsibility of the patient regardless of insurance coverage. We will always do our best to submit your insurance charges for you to maximize the benefits you do have with your insurance. There are a few insurance companies that will not pay the dental office and will only send the payment amounts to the patient. In those instances, we require full payment at the time of service and will have the insurance check sent directly to you. This would also apply to anyone presenting with Medicare Insurance. Medicare changes so frequently in what they will and will not cover it is impossible to keep up with those changes, thus we ask all Medicare patients to pay in full and again, the payment will go directly to you.

Some treatment or extenuating life circumstances may necessitate special financial arrangements. Please feel free to discuss your concerns with our front office at any time.

Insurance Information

Landmark Dental Group is a fee-for-service practice. We are not affiliated with any dental insurance company or their network. This allows us to treat patients in their best interest and not be limited by restrictions placed by an insurance company. We feel that treatment decisions should be made between the patient and their doctor, not a third party. We will gladly submit claims to most dental insurance plans and help you get reimbursed to maximize your benefits. Insurance can be complicated, please contact our office and we will assist you in this manor. We have an excellent front office team that deals with insurance daily. Many of our patients utilize some form of dental insurance and can be seen at our office.
