As a family-owned and operated practice, we work hard to create an office environment that is welcoming to patients of every age. It is our goal to not only help you and your family improve your dental health, but achieve healthy smiles for life.
Your First Appointment
Aside from providing you with advanced treatment and cleanings, you first visit to our practice is our chance to get to know you and your goals for your smile. During this appointment, we will help you lay the foundation for your long-term oral health. Our team and doctor will review your dental and medical history to ensure you receive the care you deserve.

To save you time before your initial visit, please print out and fill in the forms below.
If you are unable to fill out these forms beforehand, we recommend arriving at least 20 minutes before your appointment is scheduled to start. This will give you enough time to fill out the paperwork.
Scheduling
We work hard to maintain our schedule, because we realize your time is valuable. We request you arrive to your appointments on time so we can stay on time for all of our patients. We only deviate from our schedule if there is an emergency that needs to be taken care of immediately.
If you need to change or reschedule an appointment, please call our office at least 1-2 business days in advance.
Insurance & Payment Information
We accept most insurance plans but do not belong to any specific network. This allows us to provide each patient with the care they need, not what your plan dictates. We are one of the few — if not the only practice — in the area to provide fee-for-service dental services.

At the time services are rendered, payment is expected, unless you and our office make other arrangements. This includes the amounts not covered by your insurance (co-payments or deductibles).
